Sussex Health & Care Plan

Over the last nine months, a plan that aims to improve the health and social care of the people living in East Sussex has been developed.

As medicine advances, health needs change and society develops, so the NHS has to continually move forward so that we have a service fit for the future.

East Sussex has amongst the highest numbers of over 65-year olds and over 85-year olds in the country. Within this, many people live their later years in ill-health, often with more than one long term condition, and this is driving increasing demand and pressure on health and care services and resources.

There is a clear gap in life expectancy between people who live in the most and least deprived areas of the county; this gap is 7 years for men and over 4 years for women. improve outcomes.

Having finalised proposals to convert the Minor Injuries Unit at Lewes Hospital into an Urgent Treatment Centre, the Clinical Commissioning Group is now exploring options for expanding the offer for local people at Uckfield and Crowborough Hospitals.

The purpose of the Health and Social Care Plan is to bring real benefits to the lives of the people, through:

  • Improved, equitable access to high quality, safe and joined-up care.

  • Greater access to health and care professionals with the most appropriate skills for people’s specific needs.

  • Health and care being tailored in a more personalised way at home, or as close to home as possible.

  • People being better supported to improve their own health and wellbeing.

  • Better availability of non-clinical solutions that will keep people healthier for longer.

  • Easier interactions with health and care services through technology.

  • Greater joined-up support for people with multiple health conditions.

  • Better support to people in care homes to ensure safe and high quality care.
